The CCD is the open "standard" that's used for data interop testing at CCHIT. You're right; it takes months to understand the technical landscape/mess. Project Laika is the open source service to test CCD interop: http://laika.sourceforge.net/

Project hData (http://www.projecthdata.org/) is an attempt at simplifying the data framework.
