One needs to pony up ~$100 for access to the CCR schema... which is insane. It should be an open standard. The CCD is the open "standard" that's used for data interop testing at CCHIT. You're right; it takes months to understand the technical landscape/mess. Project Laika is the open source service to test CCD interop: http://laika.sourceforge.net/
Diabetes devices are also a special interest of mine, being an insulin pump user. Its frustrating. That's on the personal side of this whole health design thing for me. Anyway, just wanted to comment that I can relate!
